I do hereby release ownership to Phoenix Rehoming of:

I understand that upon relinquishing my dog to Phoenix Rehoming, they become the sole owner of the dog. I will
make no attempt to reclaim my dog. I fully grant Phoenix Rehoming the right to give my dog any medical treatment
deemed necessary, and to place the dog in a good foster home until an adoptive family can be found.

I HEREBY STATE THAT I AM GIVING UP MY DOG OF MY OWN FREE WILL.
